Before Your First Visit
How to get prepared for your appointment:
- Make a list of questions that you have to discuss on your first visit.
- List signs and symptoms that can be of importance to the doctor and/or therapist.
- Have in writing key past medical history, previous treatments, current medications, and other important information.
- Please call our office to verify your insurance benefits.
- Fill out our inpatient FORMS and bring them with you. Forms are also available for you in our office.
- Arrive 15 minutes early for your first appointment.
What to bring with you on the date of your visit:
- Health Insurance Card
- Photo ID.
- Completed New Patient Intake Forms (forms are also available in the office)
- All copies of medical records and imaging if you have had previous treatment for the injury for which you are being seen
- Payments for all co-pays, co-insurance, and deductibles (if applicable) will be collected when you check in on the day of your appointment
- Bring any medical assistive devices, such as a brace, splint or crutches.
What to Wear:
For your first appointment, please make sure to wear comfortable clothes, such as clothes you might wear to the gym and bring a pair of shorts with you, as you may need to wear shorts for certain types of treatments. If you have any questions in advance of your first visit, please contact us.
Patient Forms:
At NJ Pain and Rehab, we want to maximize your time with us. Therefore, we offer our paperwork online, so you can complete it in the privacy of your own home. Prior to your first visit, please download the Patient Intake Forms and bring them with you to your first visit, along with your insurance information and photo ID. Patient forms are also available at the office for your convenience.
Important information about your privacy will be discussed and additional paperwork (if any) will be completed at the facility during your first appointment.
What to expect during your visit
- You will meet our front desk staff upon your arrival. They will help you with your paperwork and update you with your insurance information, benefits, and eligibility and get authorization if needed, depending on your insurance carrier.
- COVID-19 Update: Here at NJPR we take the health and safety of our staff and patients very seriously. As a part of our safety procedure you will be asked to fill out a Covid-19 visit questionnaire, which is available online for your convenience, but has to be filled out at the time of your visit. Also your temperature will be taken at the time of your appointment and you will be requered to wear a mask inside the facility. To maintain a social distancing we ask that our patients come in alone – or if assistance is needed – to bring only one caregiver.
- You will be assigned to an experienced licensed physical therapist or other specialist to work with you.
- Your physical therapist will evaluate your condition and create a customized plan of physio care for you, based on the latest evidence of physical therapy clinical research.
- You should be expecting a follow-up call within 24 to 48 hours after your initial visit to get your feedback.
- We believe that teamwork and ongoing communication with your physician are paramount for effective patient physio care. With your permission we will be in close communication with your physician about your condition and progress as a part of our comprehensive approach.
